- 1st trimester –less than 14 weeks 0 days
- 2nd trimester- 14 weeks 0 days to less than 28 weeks 0 days
- 3rd trimester- 28 weeks 0 days until delivery
- If the provider documents gestation at 39 weeks and 6 days the code for 39 weeks of gestation should be assigned, as the patient has not yet reached 40 completed weeks
- Pre-existing condition : Pre-existing hypertension and Pre-existing diabetes “O24.0-” . “O10” series code for pre-existing hypertension
